South Eastern Community Connect (SECC) is looking for a part-time Registered Nurse with Home Care Package case management experience to help us support our community.

The successful applicant must have a current AHPRA and an unrestricted driver’s licence.

This is a part-time role (20-25 hours/week), supporting clients across Randwick, City of Sydney, Waverley, Woollahra and Bayside LGAs.

You will work in a warm, supportive and dynamic team based in Eastlakes, Sydney.

ABOUT THE ROLE

As a Registered Nurse/ Clinical Case Manager, you will join and support our dedicated Home Care Package team to provide clinical governance and guidance on appropriate care and support for our aged care clients. This role will involve, but is not limited to:

  • Applying recognised standards of clinical care when assessing and interpreting clinical indicators of general health status
  • Documenting clients’ clinical care needs and developing care management plans
  • Clinical support, training and supervision of aged care workers in clinical care management
  • Effective communication and a genuine interest in older people.

ABOUT YOU

To succeed in this role, you will bring a strong clinical skillset, be passionate about utilising evidence-based practice and thrive when working within a varied environment. Your ability to assess, plan and review clients’ clinical care needs is essential for this role.

You will assist in managing your team and provide efficient and effective case management, coordination of services (including some on-call) for clients receiving individualised home care packages.

You will maintain and update care management systems and review care plans. You will follow best practice and operate in accordance with the Aged Care Quality Standards with a focus on Standard 3. Personal & Clinical Care.

AS OUR IDEAL CANDIDATE, YOU WILL HAVE: 

  • Your current AHPRA Registration – Unconditional
  • Client focused case management experience in hospital, community or clinical settings
  • Practical understanding of the Aged Care Sector, Quality Standards and Principles, and other regulatory requirements
  • Excellent interpersonal and communications skills
  • The ability to manage a team
  • Problem-solving skills
  • Organisational skills with excellent attention to detail

Remuneration & benefits 

The salary is paid under the Nurses Award Level 4 ($39.44) – Level 8 ($47.45), and you will work across 20-25 hours per week.

As our successful applicant, you are required to be vaccinated as per the NSW Public Health order and compliance with NSW Health Compliance with Occupational Assessment, Screening and Vaccination against Specified Infectious Diseases.
